Dr. Salerno also introduces a distinction that reframes everything: the difference between defense mechanisms, which are unconscious and reactive, and offense mechanisms — proactive, intentional strategies deployed to gain advantage. Under that lens, narcissism isn't a fragile false self protecting hidden shame. It's an investment in a preferred image, built deliberately, because that image wins favor and protection when the person misbehaves.
